We start with a home support assessment to discuss the types of service you wish to receive.
You meet with your case manager to discuss your care needs, goals and service preferences and develop a care plan tailored just to you. Your case manager will also keep in touch with you and review your plan regularly to ensure it continues to meet your needs.
One of our qualified carers shows up at your place as agreed, ready to provide that little bit of help that could make all the difference to your life.
