At Home Aged Care
Many people prefer to live in their own home as they get older. Forrest Home Care provides at-home aged care to people right across the Riverina, so you can continue to live your life, at home with as much independence as possible.
At-home care may not look like you think. Yes, we can support you with personal care such as showering and dressing. But we can also provide that little bit of extra help with daily tasks such as gardening, housework, and meal preparation. Or we can simply drive you to your appointments and take care of the groceries.

Getting Care at home
You can choose to pay for private help for yourself, or you may be eligible to receive government subsidised support through the Commonwealth Home Support Program (CHSP) or Home Care Package (HCP) program.
From 01 November 2025, the Government is introducing the new Support at Home program. https://www.myagedcare.gov.au/media/283278
This will replace the Home Care Package (HCP) program from that date.
In 2027, it will also take the place of the Commonwealth Home Support Program (CHSP).
To receive government-subsidised support, you’ll need to register with www.myagedcare.gov.au and have an assessment to see what support is right for you.
Commonwealth Home Support Program
The Commonwealth Home Support Program (CHSP) is designed to give you just a little extra help to make everyday life easier and safer at home.
With CHSP, you’ll need an individual referral code for each service you’d like to receive. You’ll also be asked to make a small co-payment towards the cost of your care.
While many services are available under CHSP, our funding allows us to provide the following:
Personal Care- support with bathing/showering, continence care/toileting, and medication prompting.
Domestic Assistance - help with your everyday household tasks such as cleaning, laundry and grocery shopping when you are no longer able to do these things for yourself.
Social Support - Assistance with attending appointments, shopping, meal preparation, or joining in social activities to support your wellbeing and community connections.
Flexible Respite - companionship and care while your usual carer takes a short break.
Nursing Care - care from a Registered Nurse, including medication support, wound care, catheter and stoma care, and enteral feeding.
CHSP will be rolled into the Support at Home program in 2026. Forrest Home Care will continue to provide CHSP services until it is rolled into the Support at Home system at which time we will transition CHSP clients to Support at Home.
Support at Home
The Support at Home or SAH home care system commenced on 01 November 2025. SAH replaced the Home Care Package (HCP) Program and the Short-Term Restorative Care (SRTC) Program.
Forrest Home Care provides a wide range of coordinate cares and services to meet the assessed needs of eligible older people. Our Support at Home services can be broadly grouped into the following categories:

Our Care Partners will help you understand your assessed service level and your service budget. We will help you decide how best to utilise your assessed funding
We will help you consider any un-funded service or services requiring a co-contribution to build a service package that is affordable and best suits your needs.
Support at Home services are grouped into three Service Types: Clinical Supports, Independence, and Everyday Living as detailed below.
Clients who were receiving services under a Commonwealth Home Care Package (HCP) before 01 November 2025 will be automatically transitioned to Support at Home.

Clinical supports
Specialised services to maintain or regain functional and/or cognitive capabilities. Services must be delivered directly, or be supervised, by university qualified or accredited health professionals trained in the use of evidence-based prevention, diagnosis, treatment and management practices to deliver safe and quality care to older people

Allied Health and Therapeutic services Allied Health assistance, Counselling, Physiotherapy, Dietician, Nutritionist, Exercise Physiologist, Music Therapist, Physiotherapist, Podiatrist, Psychologist, Social Worker, Speech Pathologist, Aboriginal and Torres Strait Islander health practitioner / health worker

Independence
Support delivered to older people to help them manage activities of daily living and the loss of skills required to live independently.
Personal Care Assistance with self-care and activities of daily living. Assistance with self-administration of medication. Continence management (non-clinical)
Social Support and Community Engagement Group and Individual Social Support, Accompanied Activities, Cultural Support, Digital Education and Support, Assistance to maintain personal affairs

Home Care Packages (HCP)
The Commonwealth Home Care Package program ceased on 31 October 2025. Existing Forrest Home Care HCP clients were transitioned to the Support at Home Program on 01 November 2025, with the introduction of the Aged Care Act 2024 on this date.
If your income is above a certain amount, you will be asked to pay a contribution called an Income Tested Care Fee. This is determined by completing an income-assessment through Services Australia. https://www.servicesaustralia.gov.au/sa456

Self-Funded Private Care
From a little extra help to managing complex health needs, Forrest Home Care also offers home care options for people who do not meet eligibility requirements for government subsidies.
You can book one-off or regular private services in addition to any government-funded services you receive.

We offer a range of services on a short-term or ongoing basis depending on your needs. The types of services you can access include but are not limited to:
Personal care: assistance with personal activities such as bathing, showering, toileting, dressing and undressing, mobility and communication.
Domestic assistance: assistance with housework, laundry and home maintenance.
Nutrition, hydration, meal preparation and diet: assistance with preparing meals, including special diets for health, religious, cultural or other reasons; assistance with using eating utensils and assistance with feeding.
Mobility and dexterity: providing crutches, walkers, walking sticks, mechanical devices for lifting, tri-pillows, pressure-relieving mattresses and assistance with the use of these aids.
Social support: assistance with shopping, paying bills and attending appointments, accessing social, recreational and leisure activities to promote a sense of wellbeing and social interaction.
Nursing, allied health and other clinical services: includes speech therapy, podiatry, occupational or physiotherapy services, hearing and vision services.
Transport and personal assistance: visiting health practitioners and attending social activities.
Management of skin integrity: assistance with bandages, dressings and skin emollients.

Packages and Services
To help you to live in your own home as long as possible, the Australian Government subsidises services and products that support your independence.
Before you can be approved for care, your needs must first be assessed – for a Home Care Package.

Social Participation Program
Staying connected is an important part of healthy ageing.
Our Social Participation Program supports Forrest Community Services customers with access to social, recreational and leisure activities to promote their sense of wellbeing and social interaction.
Our customers can pick and choose which activities they take part in, such as:

Themed lunches (e.g. Christmas in July, Easter)
Day trips
Manicures
This program is available in the Griffith, Tumut and Wagga Wagga areas.
